The information you share while using this site will be used to help you get access to our health care services. Learn more about how this data is protected and used below:

  • Your responses on the online assessment: The online assessment helps you receive tailored service recommendations. Your responses to these questions are not shared with clinic staff. Summary responses from the assessment that do not include any identifiable information will be used to improve the services provided by the site. For example: “250 people who completed question 1 on the assessment answered “yes”.
  • Phone number: We require a valid cell phone number for clients to book services in order to send appointment confirmations by SMS and to receive free follow-up services from a confidential case manager, if required and if you approve. You are not obligated to receive these follow up services or communications. Your phone number is shared only with the same clinic staff who would ordinarily have access to your medical records.
  • Clinical data: any clinical data related to your visits remains accessible only to limited clinical staff who are required to access that data in order to support your service delivery and care. For example, a case manager providing follow up counseling support or who is facilitating future appointment bookings.
  • Added safeguards: This site uses many security measures to protect clients and data including using SQL, an encrypted server, and password protected staff logins for clinical and case managers.
